Conference Registration Closed!

Registration for the 2011 Updates in Sleep Medicine Conference is closed.  Please join us next year check this site for more information.

 

Register online — pay by credit card

The easiest way to register for the 9th Annual Updates in Sleep Medicine Conference is the online registration form. Registering online will streamline your payment and your records for continuing education credits. You will need a major credit card (MasterCard, Visa, American Express) to pay for your online registration. 

Register by fax — pay by credit card; Register by mail — pay by check

A printable registration form is available here. Please read the instructions carefully and complete all information. Please print clearly so we can process your continuing education credits appropriately. If you register by fax, you must pay by credit card; if you register by mail, you may pay by credit card or check. 

 

Conference registration includes:

Conference registration does not include:

  enrollment   hotel accommodations
  education materials   activities other than the conference program
  all scheduled social events   parking, but there is a special conference rate
  all scheduled meals and snacks    

Conference Registration Fees for 2011 are: 

          Physician:  $200
          Non-physician:  $150
          Resident/Fellow/Student:  $90

Cancelations and Refunds

Cancellations and deadlines will apply to conference registration fees and there are deadlines for full or partial refunds based on the cost of producing materials for the conference.  October 1, 2011 is the registration deadline to guarantee all conference materials. Requests for cancellation must be made in writing and include payment confirmation details. Refunds will be made as follows:

  August 30, 2011   100%
  September 15, 2011   50%
  October 13, 2011   10%

The Four Seasons will provide a special conference hotel rate for registrants and will make that rate available to conference registrants on a small number of rooms for extended stays before and after the conference, depending on room availability.  Please check Accommodations for very important details about The Four Seasons.  There is a metro-wide, national conference scheduled in St. Louis at the same time as Updates, so hotel room availability will be very restricted.
