Registration Fees
Conference Registration Fees for 2011 are:
- Physician: $200
- Non-physician: $150
- Resident/Fellow/Student: $90
Cancelations and Refunds
Cancellations and deadlines will apply to conference registration fees and there are deadlines for full or partial refunds based on the cost of producing materials and planning for the conference. September 28, 2012 is the registration deadline to guarantee all conference materials.
Requests for cancellation must be made in writing and include payment confirmation details. Refunds will be made as follows:

Note: The Four Seasons Hotel, the conference venue, will provide a special conference hotel rate for registrants and will make that rate available on a small number of rooms for extended stays before and after the conference, depending on room availability.
